Medicare Part D Prescription Drug Changes Coming in 2025

The Centers for Medicare & Medicaid Services (CMS) recently announced a redesign that will benefit Part D drug coverage. The changes focus on lowering out-of-pocket costs, enhancing coverage, and simplifying the overall structure of Medicare Part D.

The IRA was passed in 2022 and was designed to help meet climate goals as well as to benefit many vulnerable populations — including Medicare beneficiaries. Changes to Prescription Drug Plans went into effect in 2023 with a cap of $35 on Part D covered insulin. In 2025, three more changes arrive for Medicare plans with prescription drug coverage — with notable consumer benefits.

Here’s an overview of what to expect:

More Predictable Costs:

  • No more coverage gap (Donut Hole)! After reaching your deductible, you pay your 25% cost-sharing.
  • Annual out-of-pocket costs will be capped at $2,000, providing a more manageable financial outlook
  • Excludes Part B covered Drugs
  • This will especially benefit those with high-cost medications who don’t qualify for the Extra Help Program
Enhanced Coverage Options
 
The redesign also improves access to essential medications, including certain insulins and vaccines, by exempting them from the deductible in some cases.
 
Simplified Discount System
 
The existing coverage gap discount program will be replaced by a manufacturer discount program designed to apply discounts more straightforwardly and effectively.
 
Fairer Cost Distribution
 
Adjustments in cost responsibilities among enrollees, Part D sponsors, drug manufacturers, and CMS intend to create a more equitable cost-sharing structure.
 
Prescription Drug Payment Plans
 
Beginning in 2025, each Medicare prescription drug plans must allow members to pay out-of-pocket costs in capped monthly installments rather than all at once at the pharmacy, for certain prescription drugs costing $600 or more per month.
 
Prepare to Review your Plan
 
Now more than ever, it’s important to watch for your plan’s Annual Notice of Changes (ANOC) letter. If you haven’t received this letter by the end of September, reach out to your plan. Standalone Part D plans and MAPD prescription drug coverage may see changes in 2025.
 
Plan adjustments may include:
• Premium changes

• Updated copays on different tiers of drugs

• Drugs changing tiers or being removed from the plan
